MORTUARY SERVICES AND PRICES

 Prices quoted are set by the Society's contract with:

These prices apply only for CCMS members and only at this Mortuary. They are guaranteed to be valid at the time after-death care is rendered only if prepaid. All service arrangements are to be made directly with the Mortuary, using the "Instructions" form provided by the Society. All payments for service should be made directly to the Mortuary.

PLAN A: DIRECT CREMATION --------------------$895

Includes removal of body from home or hospital, delivery to crematory in minimum container, and cremation. Cremated remains will be provided in a container suitable for mailing and may be held by the Mortuary up to 30 days.

PLAN B: DIRECT BURIAL -------------------------$1095

Includes removal of body from home or hospital and delivery to local cemetery in minimum cloth-covered flat-top casket.

For Plans A and B, arrangements for cemetery, mausoleum, columbarium or other related services, must be made separately with the providers of such services.

PLAN C: BURIAL FOR VETERAN OR SPOUSE--$1195

Includes removal of body to Mortuary for preparation; delivery to Riverside National Cemetery or Los Angeles National Cemetery in minimum container.

If you choose to upgrade the casket for plans B or C, the Mortuary offers a one-hundred dollar ($100.00) credit toward the purchase of another casket.

FOR PLANS A, B & C, THE FOLLOWING SERVICES ARE ALSO INCLUDED: Procurement and recording of vital statistics, certification of death, securing and payment of all required permits and fees, including taxes on all goods and services, counseling on Social Security, Veteran's claims, insurance, or other benefits. Cost of travel for serving any location in San Luis Obispo County or Santa Barbara County is included in the quoted price. Special travel for autopsy, when required, is not included.

ALL THESE PLANS ARE COMPLETE, not requiring any additional charges; however, additional options are available, as detailed below.

• The original certification of death, mentioned above, stays with the County. If additional certified copies are needed, Mc-Dermott-Crockett Mortuary will supply them for an additional charge of $13.00 each (charge set by the State).
  
• Ashes may be scattered at sea for an additional charge of $100.00, or shipped anywhere in the US for an additional charge of $50.00.
  
• If a Member dies outside the Mortuary's service area of San Luis Obispo and Santa Barbara counties, there is a plan called FINIS which will cover such an event. For an additional charge of $50.00, paid at the time of prepayment, the Member will be guaranteed that all arrangements will be completely paid through networking with other service providers within the US. Prepaying with McDermott-Crockett Mortuary at the current prices will guarantee that your services will be paid for when needed. Wherever death occurs within the US, you only need to call 1-800-842-5241.

PREPAYMENT PLAN
No payment is required in advance of mortuary service. However, prices have gone up in the past and may change in the future. The only way costs can be locked in is by prepaying. McDermott-Crockett Mortuary agrees that, for Members who chose to make full payment prior to death and at the current prices, there will be no other charges required at the time of death, unless optional goods and/or services have not been paid for.

If prepayment is desired, notify the Mortuary by using the form "Request for Mortuary Prepayment Statement," or by phoning the Mortuary. Prepayment may not be made until the Mortuary has received the member's "Mortuary Instructions." The Mortuary will then send a statement indicating the amount to be paid. The Mortuary places the received payment in an insurance policy, where it is held until the requested services are rendered. The member, however, retains the right of cancellation and may request return of the prepayment at any time. This would void the guarantee against future increases. If the prepayment remains on deposit until services are rendered, the amount in the account, including earned interest, is conveyed to the Mortuary. The interest provides an inflationary hedge in consideration of the Mortuary's price guarantees.

It also permits installment payments to be made. Initiation of an installment plan freezes prices of services in effect at that time. Because the Mortuary has agreed to serve our members at a substantial savings, all charges are expected to be paid in full at the time services are rendered.
